SPARKS AYSO REGION 166 REGISTRATION INFORMATION

Early Registrations and Regular Registration for the Fall 2010 Season will be held in April and May. Please see registration schedule below with dates, times and locations. Preferred method of registering a player is pre-registering on line.

| Pre-Registration Instructions: |
| 1. Pre-Register online at
eayso.org, print 4 copies of the registration form and sign & date all 4 copies. |
2. Next, come back here to
sparksayso.org and follow the Kid Zone link to the
Kids Zone Pledge Form.
Print 4
copies for each child registered and sign & date where indicated. |
3. Bring all signed copies of registration forms and the Kids Zone Pledge to one of the registrations.
Also, bring registered player's birth certificate (a copy is fine). Registration fee may be paid
by cash or check. (If you
cannot pre-register online, make sure you bring a copy of each
birth certificate!) |
4. If you are interested in volunteering (which we highly encourage!) as a coach, referee, team mom, etc.,
your division coordinator will get you a volunteer form and get you signed up! Remember, this
organization is only as successful as its volunteers! |
5. If you are interested in volunteering (which we highly encourage!) as a board member, coach, referee,
team mom, etc., complete
volunteer form online and bring to the meeting with the coordinator. |
NOTE: There is a $5 discount for each player pre-registered on line. If a player is not pre-registered online,
bring a birth certificate (or a copy) of player(s) to be registered to one of the registrations listed below,
along with cash or check for payment. No registrations will be handled through the mail.
REMINDER: Birth certificates are required for all returning and new players –
pre-registered or not pre-registered.

There will be no change in registration fees from the previous year.
Fee schedule for 2010 is as follows:

| Regular Registration Fee: $100 per player |
| VIP Fee (division for players with special needs): $45 per player |

| $10.00 for each additional sibling registered in a family
(only applies to Early Registrations) |
| $5.00 for each player registered online |
